The City of Bloomington provides employment-related background information for former police department employees pursuant to Public Act 104-0158.
This form is intended for law enforcement agencies and authorized employers conducting a pre-employment background investigation of an individual who was previously employed by the Bloomington Police Department.

All requests must be submitted by an authorized agency representative. Requests that are incomplete or not permitted by law may be delayed or denied.
