The City of Bloomington provides employment-related background information for former police department employees pursuant to Public Act 104-0158.

This form is intended for law enforcement agencies and authorized employers conducting a pre-employment background investigation of an individual who was previously employed by the Bloomington Police Department.

Information released may include, as permitted by law:

• Investigations / Disciplinary Records

• Dates of Employment

• FTO/ Daily Observation Sheets

• Training Records

• Background Records

• Polygraph

• Psychological Exam

• Pre-employment Physical

• Resignation/Termination

• Other employment-related records authorized for release

All requests must be submitted by an authorized agency representative. Requests that are incomplete or not permitted by law may be delayed or denied.

Please allow a reasonable processing time after submission. If additional documentation or clarification is required, the Human Resources Department will contact the requesting agency directly.
